Looks like the 48th contiguous probably happened in 1968 or 1969, but the name of the 48th is not stated:
McDonald’s Hawaii ... The first location opened in Aina Haina on Nov. 2, 1968.

McDonald's first opened stores outside the United States in 1967 in British Columbia and Puerto Rico; at a time it had not yet entered all 50 states. In 1970 McDonald's opened a store in Alaska, completing its presence in all 50 states, and one in Costa Rica, its first outside North America.
