A quick review of https://en.wikipedia.org/wiki/List_of_islands_of_the_United_States_by_area suggests roughly 9.4 million people on the three major New York islands, roughly 1.2 million in Hawaii and a few hundred thousand elsewhere (together with just over 4 million in Puerto Rico, Guam, etc outside the main fifty states).
The US census website http://www.census.gov/popclock/ suggests a current population of 320 million for the fifty states. So, say 11 million of 320 million = 3.4% for the fifty states (ignoring rounding and timing).
Of course, the question could also cover Americans living abroad in the United Kingdom, Ireland, Australia, New Zealand, Japan, islands in Canada, etc, etc, etc ...
