Is the Star Wars character Yoda played by a very short person in a costume, or is he some sort of a puppet?

In the old films, he was definitely a puppet. Either hand operated or machine. In the new ones, he is probably digital animation, or some combination of digital and puppetry depending on the situation.
Frank Oz voiced the original Yoda, but I heard he died. Is it true or one of those celebrity rumors?
