Why do sockets have three holes and some plugs two pins?

On plugs with three pins, the third pin is a ground. Smaller appliances and things that don't require as much electricity don't have the ground plug. Larger appliances and things requiring a lot of electricity have the ground. Older sockets often only have two holes... and a ground socket can be added to these by screwing it into the screw holding the plate over the socket.
