Does the 'guay' ending on Uruguay and Paraguay mean the same thing?

It certainly does...
From the GuaranĂ, the language of the local people of that area, the ending "guay" means to, or from water. "Gua", meaning to/from, the simple one letter word "y" being the Guarani for "water". https://en.wikipedia.org/wiki/Paraguay
