Only four colleges have been number one seeds in 3 consecutive season:

Duke, 1998-01 (4 consecutive)
DePaul, 1980-82
Virginia, 1981-83
Oklahoma, 1988-90
Kentucky, 1995-97

DePaul did not win a game; they lost their first game in each tournament 1980-1981-1982.
