I'm not finding the name of the first Arizona test-tube baby but this link states that in 1985 Dr. Lewis Heller aided in the delivery of Arizona's first test-tube baby.

Her name was Kristian and she was born in January of 1985. Dr. Heller delivered her but she was conceived through the fertility practice of Dr. Robert Tamis, where Dr. Heller worked. Although three doctors in Phoenix were vying to produce the first baby, only Dr. Tamis was successful at first and six babies were born through IVF from January to June of 1985 including one set of twins. He later had several sets of triplets as well.
