In preventing 'root rot' in plants, what is the ratio of 3% Hydrogen Peroxide to water?

Three percent Hydrogen Peroxide may be added at up to three ml's per liter (2 12 tsp. Per gallon), but it is recommended that you start at a lower concentration and increase to full strength over a few weeks.

Note: ferfer72's answer is for dilution of 35% H2O2 which would then be utilized in myrab51's mixture of 2.5 teaspoons per gallon of water for direct application.
